The iron chest near Gavlan in No-Man's Wharf. Sold by Head of Vengarl for 5000 Souls in Shaded Woods. Acquired as a drop from Knight enemies in Drangleic Castle. Acquired as a drop from the red phantom outside of the Executioners Chariot boss (not guaranteed).
