Witcher 3 Basic Needs strives to improve realism and immersion adding a basic needs system to the game. The mod still needs some testing and maybe some balancing.

Hunger debuffs:

Level 1 starts about 6 hours after the last full meal. Stops vitality regeneration.

Level 2 starts about 3 days after the last full meal. Stops vitality regeneration. Removes a small amount of vitality per second.

Level 3 starts about 7 days after the last full meal. Stops vitality regeneration. Removes a decent amount of vitality per second.

Note that you can't meditate if your vitality is being damaged.

Thirst debuffs:

Level 1 starts about 3 hours after the last full drink. Stops stamina regeneration.

Level 2 starts about 1 day after the last full drink. Stops stamina regeneration. Removes a small amount of stamina per second.

Level 3 starts about 2 days after the last full drink. Stops stamina regeneration. Removes a decent amount of stamina per second.

Note that you can't start sprinting if your stamina is damaged.

Fatigue debuffs:

Level 1 starts about 18 hours after the last full rest. Reduces overall speed by a negligible amount.

Level 2 starts about 2 days after the last full rest. Reduces overall speed by a small amount.

Level 3 starts about 3 days after the last full rest. Reduces overall speed by a decent amount. Blocks sprinting and running.

You can change the buff names and time of effect in the configuration file inside the mod folder modW3GONeeds\content\scripts\local\W3GONeedsOptions.ws

Uninstall:

Before removing the mod make sure that you have none of the buffs and debuffs active or they'll get stuck on the character.
