Answer: Avaa's visibilty is actually event-based. The trigger is the actual item pickup in the dlc itself which is now the 4 Old Dead One's souls.

Answer: Soldier Key is in treasure chest behind illusionary wall in Forest of Fallen Giants.

Answer: Found in Iron Keep in front of fire-spitting minotaur statue.

Answer: Drops from Elana boss fight in Shulva.

Answer: drops from "Guardian Dragon" boss fight.
