Pressing Block key with Direction (Arrow) makes Geralt Dodge in given way. Now with gamepad support.

I made this simple mod to have both Block and Dodge in Right Mouse Button. This allows me faster and more precise reaction for both human and monster enemies. So now you may try this approach, too.
Mod changes playerInput.ws, so may need a merge, as this file is modified frequently. But changes are really simple, so merge should be trivial.
Mod supports gamepad.
