The latest version of the game currently available on Steam is a broken mess, fortunately it can be fixed in seconds by downgrading. With the correct package you can fix the vast majority of bugs, the game will run fairly smoothly, and you can enjoy mod support. As ever, make a backup of your game folder then install the downgrade files. Start up your game by launching straight from the PlayGTAIV.exe and you should be running at 60fps with none of the horrible stuttering. On a decent machine you can probably max the settings and be fine but I prefer to play at 30fps (how the game was always intended to run) with shadows set to high during the day and medium at night, that gives me a consistent framerate. Try to set view distance as high as possible to avoid flickering textures.

So that should be everything fixed. Now you can begin looking for mods to improve the game, so with a beefy PC you can use an enb and end up with something that looks as good as gta5, the trick here is to try different ones until you find the right one for both you and your PC. Some will hit fps really hard and others will only drop a few frames but look just as good. I have fxaa plus lumasharpen and deband enabled which for me gives zero hit to performance. Changing weapon and vehicle sounds is another economical way to improve the game, I also recommend trying different mods that change vehicle handling if you find vanilla cars unpleasant to drive. Once you find one you like, begin a new game or vehicles may not get saved in your parking space.
