This mod does require the Dawnguard and Dragonborn DLC.

I recommend Body Slide so you can run the files.  In the future may add an outfit system to enable you to customize Ithelia.  You don't need to as all NPC's are standalone.

Lore about Ithelia

Ithelia is a Daedric Prince whose realm revolves around choices, possibilities, probabilities, and consequences. Unlike her more chaotic counterparts, she is less focused on disorder and more on the intricate web of decisions and their outcomes. She thrives on influencing critical moments in time, allowing others to make their own choices but relishing in the consequences that follow. Her power lies in understanding the vast potential futures and how a single decision can ripple across reality, making her a force of balance, yet unpredictable in how events may unfold.

Beyond her role as a Daedric Prince, Ithelia holds a unique place as the Prime Archon, a member of the Magne-Ge, and the first daughter of Magnus, the creator of Mundus. This heritage gives her an immense understanding of the cosmos, further enhancing her influence over fate and potential. Her ability to manipulate the threads of destiny has made even Hermaeus Mora, the keeper of forbidden knowledge, wary of her power.

Ithelia & the Threads of Fate is a story-focused follower mod featuring five unique companions.  At the center stands Ithelia, Daedric Prince of Paths, an unwilling guide, cosmic observer, and living crossroads of possibility. Bound to the Dragonborn, she acts as a Quest Teletaxi, narrative commentator, and metaphysical anchor across Skyrim, Solstheim, and beyond.
