In the quest Et Tumor, Brute?, Caesar goes over to a chair inside his tent, sits down, tells you about his brain tumour, and then you can offer to sell Arcade Gannon to him to cure his tumour. That's all well and good, but this is the ONLY point you can give him Arcade. If you don't suggest it, don't have him as a companion, or suggest it but Arcade isn't outside Caesar's tent at this ONE time, you can't access the dialogue again even before Caesar goes into his coma, despite your quest journal having '(Optional) Have Arcade Gannon enslaved as Caesar's personal physician' in it.
This mod adds an option to offer to enslave Arcade at any point before Caesar goes into his coma.
