There will be no Journal entries! The task is simple - to destroy three powerful "Lich", each of whom has portal keys.

Having gathered all the keys (respectively killing three Liches), you will be able to pass through three portals that lead to a place that has been hidden from sight for many centuries.(basically, just like the previous ones)))

How to start the quest.

  Go to the lands of Ald Redaynia. On the northern part behind the tower you will see the entrance to the cave.

How did all this happen

"In the far western shoals of Sheogorad,where the broken stones of Ald Redaynia brood over the sea, a storm of uncommon  temper struck not long past. When the winds tore away cliff and soil, the villagers glimpsed spires long buried, and a narrow mouth yawning on the island’s north face. Since that night, sailors whisper of uneasy sounds drifting over the waters — low, choking murmurs, and the scrape of something vast beneath the hull.

Some claim pale tendrils rise from the foam to brush their vessels, neither harming nor heeding them, as though testing
