It is client-side and mainly just makes inventory highlighting look cleaner and easier to see. You can change both colors in the config and alpha/opacity/transparency works too so you can make it more subtle or more visible depending on what you want. Hover Highlight changes the background tint of the slot you're hovering in inventories, and you can also have it tint the selected hotbar slot too if you want.
