It's built around being significantly harder than vanilla, even in the easiest difficulty. Enemies are high-level, and much more likely to spawn with mod weapons, if you have any installed. Due to being closely tied to an early game quest, this means you'll have to grind and get better gear before taking it on.
Survival Mode recommended for max pain and suffering.
Performance?
With good CK practices (Not moving or deleting vanilla placed objects, only adding. Didn't add any physics objects.) I didn't notice any performance hit at all. Might vary depending on your load order.
