📋 About This Mod
IN THE FLESH - A HORROR QUEST MOD
MADNESS UNLEASHED
The second mod in the Antagonist quest mod series.
Venture into a secret Institute facility and face the evil that dwells within.
A macabre mixture drawing inspiration from Lovecraft, Resident Evil, Dead Space and who knows what else.
How to start?
Just north of the Dunwich Borers pit is a grate embedded in the rock (see images for further guidance)
What should I expect?
Scares (hopefully) - Horror is different for everyone, but I hope that you feel at least a little unnerved or jumpy.
But if you want to revel in atmosphere, a few things:
- Don't use the pipboy light (or at least, find a low powered torch style mod for it).
- Leave you companions outside.
- Ramp up the music.
Custom monsters
It's what I do. That said, this mod makes use of feral ghoul-like creatures, so any mods you have effecting ferals will effect them too.
These certainly aren't required, but a few examples include:
Zombie Walkers (https://www.nexusmods.com/fallout4/mods/20485)
Feral Ghoul Bite Skills (https://www.nexusmods.com/fallout4/mods/46097)
Horrorghouls soundpack (https://www.nexusmods.com/fallout4/mods/37419)
Also note - The enemies should level with the player, but it can still be challenging for lower level characters. In classic survival horror style, running away is always an option. And in some cases, the ONLY option.
Anything else?
- WARNING - One very short portion contains flashing lights which could tentatively be considered 'strobing'. If you suffer from sensitivity, please let me know and I will sort out a version without said lights. There is also a single reference to suicide. Fallout is a mature series for an adult audience dealing with mature themes.
- WARNING - My voice acting is still terrible.
Requirements?
Automatron, Far Harbour and Nuka World DLC.
All assets used are either vanilla or free use under CC.
Thanks to justreddz for their Hooded robe assets.

📊 Pros & Cons
✅ Pros
- Huge community trust with 1,563 endorsements and nearly 500k downloads.
- Actively maintained — last updated March 2024.
- Delivers exactly what it promises: a horror dungeon with custom enemies and scares.
⚠️ Cons
- No mention of compatibility with other quest mods or DLC — potential conflicts.
- Horror mods can be hit-or-miss; without more info, you'll have to try it to see if the scares land.
For players who want a spooky, self-contained dungeon crawl without overhauling the whole game. If you're a horror fan looking for a quick adrenaline rush, this is for you. Skip if you prefer lore-rich quests with deep storytelling — this is more about atmosphere and jump scares.
💡 Beginner Tips
- Make sure you have the latest game patch installed, as the mod was updated in March 2024 and may rely on recent updates.
- If you're using other quest mods, consider load order — place this one after major overhauls to avoid conflicts.
- Save before entering the dungeon; horror mods often have scripted events that can trigger unexpectedly.
