INSTALLATION :
Step one : Download/Install the latest DSfix update.
Step two : After setting up DSfix config to your liking, set the value for Texture overriding (texoverride) to 1 to enable custom textures.
Step three : Download the RAR file, open and extract to your tex_override folder (not tex_dump) located in the games directory. (Steam\steamapps\common\Dark Souls Prepare to Die Edition\DATA\dsfix)
Step four : So long as you've setup DSfix properly and you've put the texture in the correct folder all should be fine. (wait a few seconds on main menu screen instead of jumping straight in for the image to load)
