This is a conversion of the Eye of Vecna to Baldur's Gate 3.
The eye grafts on your right socket, with visuals (same as the Hag Eye).

[]You have truesight. (Can see in darkness and see invisible things up to 24m, also +2 Perception)

]You can use an action to see as if you were wearing a Ring of X-Ray Vision. You can end this effect as a bonus action. (Can't be made in the game, but you can already move the camera through walls)[]The eye has 8 charges. You can use an action and expend 1 or more charges to cast one of the following spells (save DC 18) from it: Clairvoyance (2 charges), Crown of Madness (1 charge), Disintegrate (4 charges), Dominate Monster (5 charges), or Eyebite (4 charges). (Clairvoyance cannot really be ported to a game without a DM)
[]The Eye regains 1d4 + 4 all expended charges daily at dawn.
[]Each time you cast a spell from the eye, there is a 5% chance that Vecna takes control of your body like a puppet. He won't release you alive.

Because many of the random properties are hard to impossible to make in BG3, and some don't make sense in this game, I adapted the the expected properties:
