This is a save just before gaining the achievement "Fancy Footwork". Hitting Gortash with pretty much anything after this save point should result in the achievement popping.
Gortash has very few hit points left and the Tav in his face has an action. Hit him. Achievement.
To load the save, just unpack it to your
\AppData\Local\Larian Studios\Baldur's Gate 3\PlayerProfiles\Public\Savegames\Story
directory (don't rename anything) and then it should be there when you choose to load a game.
