THIS IS NOT A MOD, THIS IS A TUTORIAL! The downloadable .zip file you get doesn't contain any mods, only a tutorial + template to make your own.
I made this tutorial three months ago, but since I've gotten some people asking recently about making tattoos I thought I might repost it here. The .zip contains a PDF with all of the instructions, a face template by ratchsellsfornax (uploaded with their permission) and a folder setup with everything you need to load your custom designs into the game.
